What you will do:

• Collaborate with the Transfer of Care Team to ensure efficient patient discharges.

• Focus on Pathway 1 discharges and provide urgent care to avoid unnecessary hospital admissions.

• Support Inpatient Rehab Beds and maintain optimal patient movement.

• Coordinate with acute and community settings to deliver seamless care transitions.

• Engage in multidisciplinary team meetings to enhance patient outcomes and service delivery.

Person specification:

Requirements:

• Must hold current Health and Care Professions Council (HCPC) registration.

• Requires at least 2 years of experience in the occupational therapy field.

• A car driver is preferable for flexibility across sites.
