What you will do:
- Coordinate care provision with multidisciplinary and multi-agency teams.
- Arrange and attend complex meetings relating to service user care and treatment plans.
- Review treatment plans regularly and facilitate appropriate transitions to primary care or other services.
- Manage your own caseload effectively with regular supervision and support.
- Undertake risk assessments and apply risk management strategies within mental health care settings.
- Participate in the Duty rota to ensure responsive and comprehensive care delivery.
- Work autonomously while contributing proactively to a service user-centred care model.
Requirements:
- HCPC registration as an Occupational Therapist.
- Previous experience within mental health services is essential.
- Strong communication, assessment, and multidisciplinary working skills.
