What you will do:

  • Coordinate care provision with multidisciplinary and multi-agency teams.
  • Arrange and attend complex meetings relating to service user care and treatment plans.
  • Review treatment plans regularly and facilitate appropriate transitions to primary care or other services.
  • Manage your own caseload effectively with regular supervision and support.
  • Undertake risk assessments and apply risk management strategies within mental health care settings.
  • Participate in the Duty rota to ensure responsive and comprehensive care delivery.
  • Work autonomously while contributing proactively to a service user-centred care model.

Requirements:

  • HCPC registration as an Occupational Therapist.
  • Previous experience within mental health services is essential.
  • Strong communication, assessment, and multidisciplinary working skills.
