Band 6 Occupational Therapist within Intermediate Care Team position available county-wide in Cambridgeshire UK.

What you will do:

- Deliver therapy interventions to patients on the ICT pathway.
- Provide necessary equipment to support patient needs.
- Monitor and review patient progress for optimal outcomes.
- Collaborate with the intermediate care team, aiding in team development and patient flow.
- Offer support to all patients on both step-up and step-down pathways.

Requirements:

- HCPC registration.
- A minimum of 2 years’ experience in a similar role.
- A car driver is preferable to ensure smooth travel across locations for patient consultations and team meetings.
