What You Will Do:

• Assess patients' suitability for rehabilitation and determine personalised care plans.

• Plan, implement, and evaluate rehabilitation interventions in community and home settings.

• Work closely with multidisciplinary teams to ensure cohesive, patient-centred care.

• Maintain accurate patient records and reports using EMIS and other digital systems.

• Support safe and timely hospital discharge, promoting independence and optimal recovery.

Requirements:

• HCPC registration is essential.

• Excellent communication and self-motivation to manage complex cases independently.

• Driving licence and access to a vehicle required for community visits.
