What you will do:

Provide specialist occupational therapy assessment and intervention within the Acute Medicine Team.

Work across renal and specialist medicine wards, delivering personalised rehabilitation and discharge planning interventions.

Assess patients' functional abilities, mobility, cognition, and discharge needs to support safe transitions from hospital.

Independently manage discharge planning processes, ensuring timely and effective patient flow.

Prioritise and manage a varied caseload within a fast-paced acute hospital environment.
