What you will do:

- Collaborate with the Acute Discharge Planning Team to facilitate timely and efficient patient discharges.
- Support early intervention efforts for highly vulnerable patients, ensuring their needs are addressed promptly and effectively.
- Conduct comprehensive assessments and provide tailored therapeutic interventions.
- Coordinate with multidisciplinary teams to develop and implement discharge plans that meet patients' individual needs.
- Monitor patients' progress and adjust care plans as necessary to ensure optimal recovery.
- Educate patients and their families about care plans, available resources, and self-management techniques.
