What you will do:

  • Provide specialist occupational therapy interventions to stroke patients within the community

  • Work collaboratively with the multidisciplinary team to develop and implement personalised rehabilitation plans

  • Assess patients’ functional abilities and design tailored interventions to promote independence

  • Conduct assessments in patients’ homes and community settings to ensure holistic care delivery

  • Educate patients and families on stroke recovery strategies to support long-term outcomes

Requirements:

  • HCPC registration as an Occupational Therapist

  • Experience working with stroke patients or within community rehabilitation services

  • Strong assessment, communication, and clinical reasoning skills

  • Ability to work independently within the community while contributing to a wider team

  • Commitment to delivering high-quality, patient-centred care
