Join an exciting opportunity as a Band 6 Community Rehabilitation Physiotherapist in the heart of Basildon. This rewarding locum role offers up to £27 per hour for a 3-month full-time contract. Working four shifts per week from 08:00 to 18:00, Monday to Friday, you will play a key role in supporting patients through community-based rehabilitation and the Discharge to Assess pathway.

What you will do:

• Conduct comprehensive physiotherapy assessments within community settings, including care homes and patients' residences.
• Identify patients' functional needs and establish achievable rehabilitation goals under the Discharge to Assess pathway.
• Develop, implement, and review personalised rehabilitation and treatment plans.
• Work collaboratively with multidisciplinary teams to ensure coordinated and effective patient care.
• Monitor patient progress and adapt treatment interventions to maximise rehabilitation outcomes.
• Provide support, education, and guidance to patients and their families throughout the rehabilitation process.
• Maintain accurate and timely clinical records in accordance with professional standards.

Requirements:

• HCPC registered Physiotherapist.
• Previous community rehabilitation experience is desirable.
• Ability to work independently within community settings.
• Full UK driving licence required due to travel within the community.
