What you will do:

Independently assess, treat, and plan the discharge of patients within the Community Rehabilitation service.

Use recognised outcome measures and strong clinical reasoning to guide effective treatment planning.

Manage your own caseload confidently, delivering patient-centred and engaging rehabilitation programmes.

Work collaboratively within a multidisciplinary team to support patient recovery and progress.

Apply your expertise to improve functional outcomes and independence in a community setting.
Requirements:

Registration: HCPC registered Occupational Therapist.

Experience: Post-qualification experience within the NHS, ideally in community and rehabilitation settings.

Driving: A valid driving licence is essential due to the community-based nature of the role.
