What you will do:

  • Independently assess, treat, and plan the discharge of patients within the Community Rehabilitation service.

  • Use recognised outcome measures and strong clinical reasoning to guide effective treatment planning.

  • Manage your own caseload confidently, delivering patient-centred and engaging rehabilitation programmes.

  • Work collaboratively within a multidisciplinary team to support patient recovery and progress.

  • Apply your expertise to improve functional outcomes and independence in a community setting.

Requirements:

  • Registration: HCPC registered Occupational Therapist.

  • Experience: Post-qualification experience within the NHS, ideally in community and rehabilitation settings.

  • Driving: A valid driving licence is essential due to the community-based nature of the role.
