What you will do:

• Conduct comprehensive occupational therapy assessments, including functional and environmental evaluations
• Manage complex patients with conditions such as frailty and multi-morbidity
• Facilitate hospital discharges and prevent admissions within a community-based Home First model
• Perform risk assessments and make clinical decisions autonomously as a sole practitioner
• Work efficiently in community settings, adapting to limited support structures
• Prioritise your workload effectively, even in demanding environments
• Communicate effectively and work collaboratively with multi-disciplinary teams
