What You Will Do

• Conduct thorough assessments of referred individuals to identify appropriate interventions.

• Develop and implement personalised care plans in partnership with patients, families, and carers.

• Deliver targeted early intervention strategies to support recovery and prevent relapse.

• Monitor, evaluate, and adapt care plans based on patient progress and changing needs.
