Key Responsibilities

• Carry out specialist physiotherapy assessments and deliver timely interventions for patients on the Frailty unit

• Apply acute care skills effectively in front-door admissions, rehabilitation, and similar clinical areas

• Work collaboratively as part of a multidisciplinary team to provide comprehensive care planning

• Develop targeted treatment programmes aimed at improving mobility and functional independence

• Maintain accurate clinical documentation and contribute to case reviews and healthcare meetings

Requirements

• Current HCPC registration as a Physiotherapist

• Full driving licence to support travel for home visits and between sites
