What you will do:

• Assess and treat patients in an acute frailty setting, delivering high-quality care tailored to each individual.
• Collaborate with a multidisciplinary team to devise and implement effective rehabilitation plans.
• Provide expert recommendations to improve patients' quality of life and support their independence.
• Conduct comprehensive patient evaluations, documenting progress and outcomes in line with best practices.
• Educate patients and their families about therapeutic processes and ensure they are engaged in the rehabilitation journey.

In order to thrive in this role, you must have experience in acute frailty, be registered with the HCPC, and possess a valid driving licence to facilitate patient visits and mobility solutions.
