What you will do:

Work across specialist ward environments, including elderly care wards, medical wards, and older adult mental health services.

Undertake medicines reconciliation activities to ensure safe and effective medication management.

Complete patients' own drugs assessments and support medicines optimisation initiatives.

Manage medicine discharge processes, helping to facilitate safe and timely patient discharges.

Supervise and support Pharmacy Assistants and other support staff where appropriate.

Undertake Accredited Checking Pharmacy Technician (ACPT) duties if qualified.

Provide ward-based dispensing support without routine commitment to hospital dispensary work.
