What you will do:

  • Conduct comprehensive assessments and collaborate with mental health service users to guide them to appropriate services or discharge to primary care when necessary.
  • Work closely with community resources and primary care to ensure service user and carer involvement in all aspects of service delivery.
  • Lead by example, promoting a high standard of patient care and support for junior colleagues.
  • Coordinate with external agencies such as inpatient services, social services, and the police to ensure seamless care for service

Salary range

  • £31 per hour
