Key Responsibilities:

• Provide expert occupational therapy advice to support individuals’ independence at home.

• Assess and coordinate the provision of equipment and home adaptations to ensure safety and accessibility.

• Collaborate with service users, carers, and community partners for a holistic approach to care.

• Maintain accurate, up-to-date records in line with HCPC standards.

• Work independently across various community settings, ensuring timely and effective interventions.

Requirements:

• HCPC Registration: Must be current and in good standing.

• Driving Licence: Essential for travel across multiple community locations.
