Job Description

Occupational Therapist - Adult Social Services - Response and Rehab - Richmond - £30 - £32 per hour

We are currently looking for an Occupational Therapist to join the Response and Rehabilitation Team within Richmond. The team is an integrated health and social care service for adults, primarily older people, working in partnership with the local NHS trust and the London Borough of Richmond Council.

The service offers individual short-term health and social care packages of support to help people regain their independence and wellbeing. Reablement and rehabilitation is provided on a short-term basis with a view of achieving individual goals. For people who have been admitted to hospital, the team will support a safe and timely discharge home or to a community setting.

Role responsibilities as an Occupational Therapist:

  • To perform specialist Occupational therapy assessments of patients including those with diverse presentations and complex physical and psychological conditions.
  • To take on Case Co-ordinator role for patients, this includes goal setting, co-ordinating care and support required, liaising with and referring onto internal and external agencies, point of contact for patient/carers, regular reviews, organising long term care if required/providing information on privately purchased care.
  • To design and deliver individual Occupational therapy treatment/intervention programmes based on a sound knowledge of evidence-based practice, using clinical assessment/ reasoning skills, use of appropriate outcome measures and knowledge of treatment/intervention options.
  • To facilitate patients and carers in the development of support plans, which meet their needs, identified SMART goals and covers identified risks, offering the widest possible flexibility and choice allowing them to take control of their short-term goals
  • To evaluate patient progress through person-centred reviews and alter treatment programmes if required in partnership with other professionals in the multi-disciplinary team
  • To act as triage therapist assessing referrals for suitability for input from team, contacting various agencies to obtain more information and writing support plans to facilitate discharge from hospital.

As an Occupational Therapist you must be:

  • HCPC registered
  • Eligible to work in the UK
