It is an exciting time to join a dedicated rapid response team to help prevent hospital admissions and help facilitate early discharge from hospitals. And right now, we're looking for an Occupational Therapist to join our new Rapid Response Service at Dartford, Gravesend and Swanley to help us continue to change things for the better.
DGS Rapid Response Team
The Virgin Care Rapid Response Team is based at Gravesham Community Hospital and provides an invaluable service of specialist advice and treatment for patients across our locality. We deliver an occupational therapy service for adults across the surrounding region and aim to make a difference to people's lives working within the community, clinic basis and community hospital.
As an Occupational Therapist you will join a multidisciplinary team and will work alongside dedicated and highly-skilled NHS-trained Occupational Therapists, Physiotherapists, Nurses, Rehabilitation support workers and administration team members. We are a team that looks to support one another in any way we can by holding regular meetings such as team, MDT and professional meetings.

Our new dedicate Rapid Response Team is looking for a Band 7 Occupational Therapist Team Lead. We are a multi-professional community based team aiming to promote the independence of community dwelling adults, predominantly older people. We have a pro-active approach to reducing hospital attendances and to facilitate discharge from local and national hospitals to reduce length of stay. We work collaboratively within the locality with other community services and wider teams, including Social Services and the local Hospice to meet the needs of our patients. We provide rapid response to all those referred to our well respected service using a client centred goal led approach.
You will have the opportunity to develop your skills in Team Leadership and Occupational Therapy assessment and treatment planning, moving and handling assessment/training, palliative care needs. You will have an active leadership and supervisory role and will develop your skills in these areas whilst supporting qualified practitioners, students and support staff. You will be responsible for the development of the service and in helping to identify the future needs of the collective services locally.
The role will also involve:
- Delivering of high quality, individualised, evidenced based care to all patients
- Ensuring that therapy input is integrated into the patients' overall care plan
- Operating as an autonomous practitioner working without direct supervision and at times lone working
- Working with managers to develop the service in line with patient need, local and national developments
- Participating in continuing professional development.
Essential to the role is a current and recognised Occupational Therapist degree/qualification, Health and Care Professions Council registration, Royal College of Occupational Therapists membership and excellent IT skills etc. (See job description for further details).
