Medicines Optimisation Pharmacy Technician
We have an exciting opportunity for a dynamic and capable pharmacy technician to join a team of experienced primary and secondary care pharmacy teams within Guys and St Thomas NHS Foundation Trust who, through a range of innovative projects, are driving medicines optimisation across Lambeth and Southwark.
You will work collaboratively with the Pharmacy teams to support the Community Nursing teams to optimise medicines related aspects of care for vulnerable adults with long term conditions in the community. Many of these patients will be over 65 years, have complex co-morbidities and be taking a number of medicines. This will include contributing to developing and implementing safe and effective medicines systems and processes within the teams as well delivering targeted support to patients with high risks of medication related problems in domiciliary settings.
You will undertake assigned medicines related tasks and facilitate implementation of safe handling and administration procedures including medicines reconciliation, follow-up medication reviews, safe selection and use of compliance aids as well as training staff. You will also be the point of contact for day to day enquires from GPs, practice staff, health and social care staff and patients on practical aspects of medicines use
We are looking for an experienced pharmacy technician with proven experience of working in partnership or collaboration with health and social care multidisciplinary teams and across organisations. The post requires you to be enthusiastic and highly motivated, and be able to plan and manage your own time to meet the needs and goals of the service.
The role will involve regular travel across the local area so you must be able to meet the travel requirements of this post.
