Maxxima is currently seeking an experienced Band 4 Community Care Assessor to fill a full time role based in Liverpool. The ideal candidate will have a significant understanding of Mental Health practices.
The starting pay rate for this locum role is £12 per hour (Umbrella Rate) and is 1 month initially with the possibility of extension, to start on October 1st.
The job role duties cover:
*To manage own case load, determining priorities
*To carry out assessments, including comprehensive initial assessments, with patients and / or their carers to determine the care needs, level of risks considered and priority assessed, in order that a variety of options can be considered and an individual care plan created.
*To carry out assessments in a variety of settings
*To offer advice, guidance and options of care provision with patients and or their relatives. Taking consideration of patient's acute and chronic physical and psychiatric conditions, some with a terminal diagnosis.
*To be part of the Multi-Disciplinary Team process
*To act as an alerted in the cases of Adult and child abuse
*To assess properties where applicable for possible cleansing services.
*To coordinate, monitor and review care plans both for long and short term provision of care.
Requirements of the role:
