Role Summary:

This post offers an exciting opportunity for an experienced Physiotherapist to consolidate their clinical skills and develop leadership skills within TLCO community therapy services.

You will be a dynamic, professional and enthusiastic team member able to work autonomously and facilitate the development of those around you. You will be joining a well-established therapy service that provides services in client's homes and other community settings.



Key Responsibilities of the Post:

• To provide a specialist patient centred physiotherapy service to those who are referred to the community rehabilitation service.

To undertake the assessment and clinical diagnosis of patients using investigative and analytical skills; formulating an individual management and/or treatment programme according to the patients goals using advanced clinical reasoning and a range of treatment skills, including knowledge of alternative treatment options.

To undertake the continuous assessment and evaluation of treatment programmes to ensure maximum patient benefit, and modify them according to the patient's needs. Ensuring timely discharge.

To demonstrate a high level of manual handling skills.

To assess and manage clinical risk and accept clinical responsibility as an autonomous practitioner for a designated caseload of patients, and organise this efficiently and effectively with regard to clinical priorities and use of time.

To utilise complex communication skills to be able to overcome barriers to communication.

Provide advice and education to patients and carers in conjunction with the multi-disciplinary team and signpost to appropriate agencies and charities.

To be responsible for the safe maintenance and competent use of all equipment, appliances and aids and ensuring each is safe for its purpose prior to use.

To be aware of, and comply with, organisational and departmental policies and procedures and to be involved in reviewing and up-dating as appropriate.

To work in support of and deputise for the Senior Practitioner in the day to day running of the team; service evaluation and design.

To be actively involved in research and audit to enable informed judgements regarding patient's care and service development to be made.

To ensure that non-registered / junior staff are clear about their responsibilities and accountability and that they have received appropriate informal or formal training and supervision, and are competent to carry out any duties that are delegated to them.

To maintain own continuing professional development and develop current knowledge of evidenced based practise.

To support the development of a culture of accountability and governance at all levels




Key Requirements of the Person Specification:

• Diploma/Degree in Physiotherapy
• HCPC Registration
• Evidence of post graduate study
• Appropriate relevant post grad experience working in a community setting
• Evidence of working in a multi-disciplinary team
• Evidence of working with clients in the community setting
• Knowledge of current best practice
• Good level of skill in treating patients
• Good clinical knowledge base
• Evidence of working with people with multiple needs and long term conditions
• Ability to undertake appropriate therapy assessments relevant to the clinical speciality
• Able to use clinical reasoning to justify relevant therapy diagnosis and formulate treatment plans
