Job Description

An exciting opportunity has arisen for a Patient Pathway Co-ordinatorto join the Neurofibromatosis (NF) Service at Guys Hospital for one year maternity cover. The department is funded through NHS England highly specialised services to provide the national Complex NF1 service from 1st April 2009 and the national NF2 service from 1st April 2010. These services have been nationally commissioned to provide specialist support for this significantly complex group of patients.

We are looking for a dynamic and hardworking individual to join our team. The role demands excellent prioritisation skills, problem-solving abilities and a collaborative approach to ensure the smooth and efficient running of administration. The post holder will work closely with consultants, clinical nurse specialists, secretaries and administrators within the team.

The aim of thepathway co-ordinatorwill be tolead on the trackingof the pathways for our patients who cometo ourunitunder our multidisciplinary team clinics. The post holder will be the main point of contact forall MDT administrative querieswithin in the team.

The main expectations of the role are:
  • Co-ordinate and provide administrative support to the relevant teams to ensure that all the relevant paperwork and clinical information is available.
  • Work within a multi-disciplinary team in developing the service in line with departmental plans and Trust corporate objectives.
  • Running our MDT clinics ensuring all patients pathways and referrals are administered in line with timings and requirements as set out in trust and national polices.
  • A central point of contact for communication with the clinical team and patients.

This opportunity will give someone the chance to develop their skills within a busy outpatient department. This role is a part time role of 22.5 hours a week.
