Job Description

We have an exciting opportunity for a fixed term contract for a Band 7 Physiotherapist and Occupational Therapy post until the end of the financial year to work as part of a multidisciplinary in-reach care home team. Improving clinical input into care homes and tailoring care around the diverse needs of individual residents can improve quality of care and quality of life for people and reduce unnecessary admission.

We are looking for a highly motivated, innovative, flexible Physiotherapist to join a new team providing community in-reach programme for older adults in care home settings. The team is part of a wider Older people’s service alongside day hospital and community therapy team based at St John’s Therapy Centre and Queen Mary’s Hospital. You will be supporting the ongoing integration project and rapid response service

The core team include Physiotherapy, Occupational Therapy, Dietitian, Clinical Nurse Specialist, Doctors, Speech and Language Therapist,.

You will be providing assessment and physiotherapy intervention for a caseload of patients with a variety of older people’s pathologies as well as providing support and education to empower care home staff. You will be a key team member in supporting the development of the service with opportunities to lead on projects, initiatives and audits

There is a strong emphasis on personal development and diverse opportunities for learning are actively supported. You will be expected to supervise students on placement.
