Bolton NHS Foundation Trust provide services across the north west side of the Greater Manchester area ranging from hospital services in Bolton to community services across Bolton, Salford, Ashton, Leigh and Wigan. We are a progressive, expanding organisation with great ambition around improving quality, integrating pathways and delivering high levels of productivity. We have a CQC rating as good overall and we have some key areas of outstanding practice with an outstanding rating for being well led at every level.
Over 5,700 people work for the Trust and many of them have been here a long time. People find they don't need to go elsewhere to find a fulfilling career in healthcare. The Trust offer a range of flexible working patterns to enable our staff to balance their work and personal life.
An exciting opportunity has arisen for a Pharmacist to join our Intermediate Care (IMC) pharmacy team in community services. The IMC tier Pharmacy Team plays a significant role in supporting patients following an acute hospital discharge or to prevent avoidable admissions to hospital during periods of acute illness.
The role will include working as part of a multidisciplinary team, including GPs, Consultants, Nurses, Pharmacy, Support Workers, Therapists and colleagues within Local Authority. We also work closely with independent providers of care providing training around medication.
The IMC Tier Pharmacy team also works closely with Community Pharmacy, CCG's and Bolton NHS Foundation Trust. We recognise that effective collaboration is the cornerstone to the safe and effective use of medicines and therefore more positive patient outcomes.
The objective of the service is to offer patients the best opportunity to maximise their potential for regaining independence, and therefore relies on an integrated and committed multidisciplinary team to deliver the shared goal.
The successful candidate will support safe and cost effective delivery of pharmaceutical services to patients in bed based services and domiciliary care where appropriate, working closely with other teams to help optimise medication management within our service.
The ideal candidate will be a highly motivated individual with a willingness to work independently and with other health care professionals at all levels. They will be an excellent communicator and have established IT skills. All applicants must have current UK professional GPhC registration and evidence of continuous professional development
The post holder will work across a number of sites and must have access to a suitable vehicle to be used within the role.
Applicants should have a minimum of 2 years post registration experience.

The vision of this Trust is to be recognised by the people of Bolton and beyond as an excellent provider of health and care services and a great place to work. We are striving to attract, recruit and retain high calibre skilled staff which reflect our communities. We are an inclusive employer who trains and develops our staff in a positive learning environment. We aim to be consciously inclusive and therefore applications from all sectors of the community are welcomed. We would particularly encourage candidates from a black and minority ethnic (BAME) background and candidates with health conditions or impairments.
From time to time we may need to close a vacancy early when in receipt of a sufficient number of applications. If you are interested in this post and wish to be considered, we strongly advise you to complete and submit your application as soon as possible.
