An exciting opportunity has arisen for a Pharmacist to join our Intermediate Care (IMC) pharmacy team in community services. The IMC tier Pharmacy Team plays a significant role in supporting patients following an acute hospital discharge or to prevent avoidable admissions to hospital during periods of acute illness.
The role will include working as part of a multidisciplinary team, including GPs, Consultants, Nurses, Pharmacy, Support Workers, Therapists and colleagues within Local Authority. We also work closely with independent providers of care providing training around medication.
The IMC Tier Pharmacy team also works closely with Community Pharmacy, CCG's and Bolton NHS Foundation Trust. We recognise that effective collaboration is the cornerstone to the safe and effective use of medicines and therefore more positive patient outcomes.
The objective of the service is to offer patients the best opportunity to maximise their potential for regaining independence, and therefore relies on an integrated and committed multidisciplinary team to deliver the shared goal.
The successful candidate will support safe and cost effective delivery of pharmaceutical services to patients in bed based services and domiciliary care where appropriate, working closely with other teams to help optimise medication management within our service.
The ideal candidate will be a highly motivated individual with a willingness to work independently and with other health care professionals at all levels. They will be an excellent communicator and have established IT skills. All applicants must have current UK professional GPhC registration and evidence of continuous professional development
The post holder will work across a number of sites and must have access to a suitable vehicle to be used within the role.
Applicants should have a minimum of 2 years post registration experience.
