The community mental health team service in Dartford, Gravesend and Swanley is a Multidisciplinary team providing services for adults of working age group diagnosed with mental health disorders.

The teams consist of 3 full time consultants, each consultant providing care for each of the 3 localities; Dartford, Gravesend and Swanley.

The Swanley consultant also provides consultant cover for the triage/duty team and the Early Intervention Team (Dartford locality).

This is a 10PA per working week post within the Gravesend locality, providing senior medical input to the community mental health service, including providing initial assessment, offering follow up reviews for patients requiring short-term input from the team and CPA Reviews for patients with long-term needs on the enhanced CPA Pathway.

The post holder will have a joint caseload with other clinicians in the team and they will be expected to work with other clinicians in keeping with the Trust Lone Working Policy
