We are seeking a dietitian with experience of providing nutrition support to patients at risk of malnutrition in the community. This post requires excellent communication skills as you will be working with a wide range of patients, carers and health professionals to provide advice, training and individualised care plans for patients referred to the service.
The LAMP team consists of x2 advanced dietetic assistants, x4 band 6 dietitians and a team lead dietitian with administrative support.
The team is at the forefront for providing nutrition support to vulnerable patients in Lambeth and Southwark and ensuring that frontline healthcare teams have the skills to identify and treat patients at risk of malnutrition.
TheLAMPteam is part of the Integrated Care service operated byGuy's and St. Thomas' NHS FoundationTrust in Lambeth and Southwark andworks alongside neighbourhood nursing and rehab therapy teams to ensure patients at risk of malnutrition are receiving optimal nutritional care.
You will be seeing patients in a home setting andoutpatient GP clinics. You will also be working closely with our prescribing support dietitians and the acute hospital dietitians to ensure that patients are receiving adequate follow-up following hospital discharge.
In addition, you will play a fundamental role in improving patient pathways of care using a MDT integrated approach to support patients with complex medical needs in the community; this includes providing specialist dietetic input for patients with long term conditions and frailty.
You will also be providing training to a wide range of healthcare professionals and carers on malnutrition screening and nutrition support.

We are a large Academic Health Sciences Centre with King's College London and King's College Hospital NHS Foundation Trust. This is a unique opportunity to work alongside experienced clinicians and leading researchers in the Trust and at King's College London.
You will be joining a department of well over100 staffworking across Guy's, St Thomas', Evelina Childrens Hospital and Lambeth and Southwark community services. The department has a strong commitment to staff development.
