Key Responsibilities:

Conduct comprehensive functional assessments for adults with physical, sensory, or cognitive needs.

Create and implement individualised care plans that promote independence and quality of life.

Undertake manual handling risk assessments , ensuring safety in the home and community settings.

Prescribe and monitor the use of simple and complex equipment , and recommend adaptations to living environments.

Evaluate housing needs and contribute to void assessments and major adaptation planning .

Maintain accurate records and ensure compliance with the Care Act , Equality Act , and other relevant legislation.

Engage with service users, carers, families, and a multidisciplinary network to coordinate services effectively.

Candidate Requirements:

HCPC-registered Occupational Therapist

Minimum 2 years' experience in a local authority or social services setting

Confident in handling a complex caseload involving adaptations, manual handling, and equipment provision

Valid UK driving licence and access to a vehicle (community travel required)
