What you will do:
* Conduct comprehensive assessments of patients in their homes or suitable community settings
* Develop and implement tailored rehabilitation programs focusing on stroke recovery

* Engage with patients and their families to set and achieve rehabilitative goals
* Maintain accurate records and documentation in line with NHS standards
* Use your car to travel efficiently between appointments and manage your schedule effectively
This role requires a car driver due to the community nature of the work, ensuring fast and flexible access to clients. Applicants must have a minimum of 1 year's UK NHS experience and be HCPC registered to apply.
