What You Will Do

Manage a caseload of service users with urgent and complex needs.

Undertake Occupational Therapy specialist assessments in clients' homes to determine eligibility and risk.

Develop tailored, outcome-focused support plans to promote independence and prevent hospital admissions.

Recommend and arrange for the provision of equipment and home adaptations , including major adaptations where required.

Work collaboratively with social workers, care providers, and other allied professionals to ensure coordinated service delivery.

Maintain accurate case records in line with organisational policies.

Requirements

HCPC registration as an Occupational Therapist .

UK-based OT experience within Adult Social Care or Local Authority settings .

Full UK driving licence and access to a vehicle (home visits required across Ealing).

Strong knowledge of relevant legislation (e.g., Care Act 2014, Housing Grants, and Disabled Facilities Grants).

Ability to work independently and manage a busy caseload effectively.
