Join a leading, award-winning domiciliary care provider and make a real difference every day. We're looking for a dedicated and experienced Care Coordinator to become an integral part of our passionate team based in Bolton.

• Creating and managing staff rotas to ensure seamless service delivery
• Collaborating with hospitals, GPs, social services, and families
• Conducting initial assessments and regular care reviews
• Participating in on-call duties and scheduled weekend coverage
• Supporting compliance with care quality standards and internal procedures

• Experience: Proven background in domiciliary care is essential
• Skills: Excellent IT proficiency, strong organisational ability, and a genuine passion for person-centred care
• Requirements: Full UK driving licence and access to a vehicle
