What you will do:

- Provide a specialised clinical service to designated wards, meeting professional, departmental, and Trust policies.

- Undertake medicines reconciliation to ensure accurate and complete medication records.

- Review medication for safe and effective use, adapting as necessary.

- Manage clinical handovers, updating them based on patient responses, investigation results, and any adverse effects.

- Conduct therapeutic drug monitoring for specific medications upon request.

- Educate patients on their drug therapy, empowering them with the knowledge they need.

- Facilitate the implementation of local and national guidelines, ensuring best practices are followed.

Requirements:

- Registered as a Pharmacist with the Health and Care Professions Council (HCPC).

- Must have access to a vehicle and hold a full driver's licence for travel between sites.
