What you will do:
* Manage a clinical caseload of individuals with complex multiple pathologies, including psychological, neurological, emotional, behavioural, social, or age-related issues
* Collaborate closely with the Complex Occupational Therapy Service team to ensure comprehensive care
* Implement treatment plans tailored to individuals' specific needs and goals
* Monitor and evaluate treatment progress, adjusting plans as necessary
* Educate patients and families on therapeutic processes and ongoing management strategies
* Maintain detailed records of patient progress and communication with relevant professionals
In this position, you will need to be HCPC registered and possess a valid driving licence as a car driver is required to visit clients across Hammersmith.
