What you will do:
* Conduct thorough assessments of patients needs and develop effective discharge plans.
* Collaborate closely with multidisciplinary teams ensuring seamless patient transitions from hospital to home.

* Maintain accurate records and documentation to support patient care and discharge processes.
Requirements:
* Registration with the Health and Care Professions Council HCPC is essential.
* You'll need at least two years of experience primarily in assessment and discharge planning preferably in a busy acute medicine setting.
* A broad clinical experience in acute environments will be advantageous along with a preference for car drivers to travel efficiently between sites.
