Job Description
The main duties of this post are:
- Coordinating the pharmacy homecare service, prioritising, and managing the workload and staff within the service.
- Attendance at MDTs, counselling of homecare patients/ their carers on homecare service provision and the use of medicines. Offering support and advise to clinical teams on homecare services offered.
- Ensure safe and effective service provision by engaging with Homecare providers, investigating incidents, and reviewing both internal and external performance indicators.
- Identify areas for service development and participate in quality improvement including with the new Electronic Patient Record roll out across MFT including homecare.
