Job overview

This is an exciting opportunity to become part of a new service based at Kent and Canterbury Hospital supporting people who have a diagnosis of dementia or cognitive impairment; who are experiencing an escalation of those symptoms or a degree of crisis requiring specialist intervention to aid their transition of care from hospital to a short term residential or nursing home establishment.

In addition to working with the MDT in the Kent and Canterbury, William Harvey and QEQM hospitals you will be required to work alongside carers and other external agencies.

The East Kent Rapid Transfer Dementia Service covers all three of the East Kent Hospital University Foundation Hospital sites and supports transitions of care from the these sites to the East Kent community assessment beds.

Main duties of the job

Under the direction of a Locality Services Manager to provide high quality mental health services to Older People with Mental Health Needs within a defined geographical area. Provide clear and timely assessment of patients with complex needs and behaviours seen as challenging; particularly dementia. Offer advice and consultation to patients, acute hospital teams, fellow professionals, carers and care home staff, in relation to managing behaviours perceived as challenging, to help promote an improved quality of life and person centred care. To assist timely acute hospital transfers, prevent unnecessary re-admissions and transfers between care home establishments.

Provide clear recommendations in relation to onward support and care jointly with the Rapid Transfer Services (KCHFT) and Kent County Council colleagues, where co -morbidities and complex physical and mental health needs are present.

Person specificationQualificationsEssential criteria
  • Appropriate professional qualification
  • Registered

Desirable criteria
  • Practice educator / nursing mentor
  • Other relevant training / qualifications


KnowledgeEssential criteria
  • Understanding of dementia presentation and treatment
  • Understanding of functional mental health presentation and treatment
  • Knowledge of mental health act / capacity act
  • Demonstrates profession specific knowledge and risk management

Desirable criteria
  • Person Centred Care


Skills and ExperienceEssential criteria
  • 2-3 years post grad experience
  • Experience working with dementia / mental health conditions
  • Prioritising and managing own workload
  • Team working
  • Working in a health or social care setting
