Job Description
To support the development and monitoring of Homecare services for medicines throughout the Trust in line with National Standards.
Working with clinical pharmacists and clinical teams, you will implement and maintain systems for ensuring NICE, NHS England and Local commissioner contract compliance.
To support the medicines optimisation relationships with commissioners and primary care.
To work alongside senior Trust, NHS England & primary care colleagues to support and evaluate the appropriate use of high cost medicines
To support access to medicines and the introduction of medicines onto the Trust formulary through liaison with medical and pharmacy clinical teams for the benefit of patients.
Using expert clinical and commissioning knowledge to identify appropriate routes of approval for individual patients.
To validate and monitor reporting for the effective management of medicines usage and financial budgets.
To support the Medicines Optimisation team to improve medication safety within the trust and to ensure good clinical governance and risk management within medicines management activities.
You will participate in an annual appraisal with the Lead Medicines Optimisation Pharmacist for high Cost Drugs and homecare and meet regularly with them to deliver agreed strategic and operational objectives.
You will contribute to the weekend and evening rotas and be able to travel within the North West to attend meetings and to work from our different sites.
