Job Description

Job overview

The post holder will be ensuring a high quality service is provided and care is delivered to all clients on the caseload, who are experiencing Mental Health Crisis, in accordance with the Trust's policies and procedures and the care programme approach. They will contribute to the implementation of clinical, and interdisciplinary strategies to ensure the delivery of a high standard of care and service objectives. They will contribute to the overall corporate objectives of the organisation. The post holder will work as a member of the Crisis Resolution Home Treatment Team ensuring team clinical performance is optimised and meeting agreed KPIs. They will ensure that CRHT is a responsive service providing timely assessment and offering a range of home treatment interventions as an alternative to hospital admission. The post holder will have robust working relationships with colleagues within acute and care such as patient flow; liaison psychiatry, place of safety, inpatient teams so seamless acute care is provided for service users. The post holder will work in collaboration with colleagues from the community recovery care group, the local authority, primary care and other key stakeholders to ensure effective care delivery and timely onward care at point of discharge.

Main duties of the job

You will have excellent interpersonal skills that you'll use to support the team to provide the best possible care. The Band 7 roles within the team share operational and clinical duties based on individual job plans.

You will work clinically across all aspects of the service and support decision making around complex cases as a key member of the multidisciplinary team.

You will provide clinical and managerial supervision to other clinicians in the team. A flexible and can do attitude is essential.

A diverse post qualification experience in mental health is required, as is at least 3 years post qualifying experience, with at least one of these years having worked in a Band 6 role.

Detailed job description and main responsibilities
  • To work as a core member of the Acute Inpatient and Crisis Resolution Home Treatment Service, ensuring clinical care is of the highest standard and meeting agreed KPIs and National Fidelity Standards.
  • To attend MDT meetings, CPA reviews and other patient reviews to ensure effective delivery of acute care.
  • To work clinically to support junior staff with complex patients and decision making
  • To ensure accurate information is collected and reviewed to ensure CHRT are meet agreed KPIs.
  • To contribute to the review of patient pathways and processes to support the reduction of inappropriate referrals, admissions and delays in discharge.
  • To demonstrate professional leadership both internally and externally to the Acute Care Group
  • Implement and maintain clinical audit and evaluation systems to ensure effective functioning of CRHT and support and signposting services meeting Clinical Governance requirements, including leading on the CLIQ checks.
  • To work with acute and community colleagues to ensure the effective communication and interface to optimise acute care.
  • To contribute to the development of help and promote the partnership working between and within primary/secondary care, and Local Authorities and third sector organisations.
  • To participate in the appraisal, supervision and talent management reviews.
  • Examine and introduce new models of care as appropriate, evaluating and existing practice and contribute to the development of new ways of working which improves service delivery.
  • To anticipate and identify clinical gaps and pressures in the service and propose solutions.
  • Deal effectively with clinical complaints, SI's, safeguarding and RCA's within agreed time limits.
  • Act as a resource and provide expert advice for the staff on issues around CRHT.
