Job Description

Main duties of the job

We are looking to recruit highly skilled occupational therapist to a post to the North Manchester Community Mental Health Team based at Hexagon Tower, Crumpsall . The successful candidate will work within an integrated, multi-disciplinary team providing assessment, treatment and discharge planning of the clients we serve. The successful applicant will be required to provide care coordination in line with the Trust Care Programme Approach (CPA) for a limited number of clients, who experience functional and/or organic mental health problems.

Detailed job description and main responsibilities
Self care and diet- assessment and recommendation for packages of care in the areas of personal care and kitchen activities, health promotion around diet and access to community resources.

Psychological health- assessment to inform diagnosis, recovery based interventions, reminiscence work, life history work, brief interventions on anxiety management, mindfulness and relaxation.

Safety to self/others-falls risk intervention, mobility and transfer assessment, advice about assistive technology/tele-care referrals.

Accommodation-home safety assessments, fire safety advice and referral, access issues, stairs assessments, provision of equipment, referral for home adaptations.

Daily occupation, structure and interests-graded programmes to return to valued occupations, adaptation of occupation for stage of life, lifestyle assessment, organising time and activity, meaningful goal setting, re-engaging with roles, improving motivation.

Social contacts-social inclusion advice and support, recovery orientated community involvement opportunities.

Close relationships-carer education, advice, support, and training in use of equipment, communication techniques, understanding behavioural changes.

Language and culture-assessment and advice on communication difficulties, referral to other SALT for complex needs.

Physical health-assessment and advice on sensory impairment, referral to the sensory team for complex needs, energy conservation advice and provision of equipment.

Looking after the home-assessment of domestic activities, recommendations for additional support.

Employment-vocational assessment, volunteering opportunities, productivity in later life.

Alcohol and drugs-health promotion, re-engagement with valued occupations following substance use, referral to the drug and alcohol team where appropriate.

Educational needs-the value of life long learning, support and advice on opportunities for training, promoting the older adult as a source of knowledge and experience.

  • Post registration experience

Qualifications

  • Degree/diploma in Occupational Therapy
  • Evidence of post qualification CPD


Desirable criteria
  • Post registration qualification in OT/mental health
