Job Description

Main duties of the job

Occupational therapy assessment, formulation and planning.

Care co-ordination and risk management of a range of complex cases.

Duty and weekend working on a rota basis.

Multi-disciplinary team working.

Arranging care packages as required as part of the care co-ordination role in accordance with The Care Act.

Detailed job description and main responsibilities
An exciting vacancy has arisen for a keen and motivated Occupational Therapist. The post-holder will join a well established and expanding multi-disciplinary team, which will be based at Breightmet Health Centre in the North of Bolton. This team provide a specialist dementia service to individuals, carers and various agencies across the Bolton Borough.

We are looking for an enthusiastic and forward thinking Occupational Therapist, who is keen to develop quality services and best practice in Older Adult Mental Health. We would welcome applicants with innovative and creative ideas, which would support the development of this service. You will occasionally be required to work extended hours including evenings, weekends and bank holidays on a rota basis. You will also be required to work duty on a rota basis.

The post-holder must be committed to professional development through, support, training and supervision, which will be essential due to the evolving nature of this service. Line management supervision will be provided by the Team Leader and Professional supervision will be provided by the OT Professional Lead.

Skills and Abilities
Essential criteria
  • Ability to work autonomously and set priorities in a demanding service environment


Desirable criteria
  • Experience of effective multi-disciplinary working


Education/ Qualifications
Essential criteria
  • Degree/ Diploma in Occulational Therapy


Experience
Essential criteria
  • Significant post registration experience as an Occupational Therapist


Desirable criteria
  • Experience of working with Older Adults mental health services including dementia care


Skills and Abilities
Essential criteria
  • A willingness to undertake the role and duties of care co-ordination within the OT role


Desirable criteria
  • Experience of care co-ordination work alongside occupational therapy work


Other requirements
Essential criteria
  • Willingness to work on duty and weekend work on a rota basis

  • A willingness to support planning, development and evaluation of the OT service in Bolton
