Job Description

Detailed job description and main responsibilities To advise the practice on the implications of prescribing and admission/referral data provided by the Clinical Commissioning Group (CCG).
To provide prescribing advice and support to the practice(s), on all medication issues.
To implement change in prescribing behaviour, where required, in order to promote quality, rational, safe and cost-effective prescribing.
To promote prescribing in line with national and CCG guidelines and policies.
To actively work with the CCG in order to optimise the quality of local prescribing of:
Antimicrobial medicines;
Medicines which can cause dependency;
Metered dose inhalers, where a lower carbon device may be appropriate; and Nationally identified medicines of low priority
To identify and advise on any areas of clinical risk connected with prescribing.
To undertake structured medication reviews and therapeutic specific medication reviews, including but not limited to reviews focussed on :
Care homes;
Those with complex and problematic polypharmacy, specifically those on 10 or more medications;
Medicines commonly associated with medication errors;
Severe frailty, particularly isolated or housebound patients, or who have had recent hospital admissions and/or falls
Potentially addictive pain management medication

Person specification Qualification Essential criteria
  • Pre-registration training and experience.
  • Registered with the General Pharmaceutical Council as a Pharmacist in the UK (or eligible to be)
  • Degree in pharmacy

Desirable criteria
  • Specialist post graduate qualification such as clinical diploma or equivalent experience
  • Independent prescriber


Knowledge Essential criteria
  • Good core therapeutic knowledge
  • Knowledge of and ability to apply all aspects of regulatory legislation and guidelines


Experience Essential criteria
  • Understanding of hospital pharmacy environment
  • Understanding of working effectively within multidisciplinary groups
  • Proven experience of delivering responsibilities and objectives to time in previous posts
