Outstanding opportunity for Medical Case Managers / Rehabilitation Case Managers to join an expanding organisation. Applicants must have a recognised qualification in Physiotherapy, Occupational Therapy or Nursing. In addition to this you must have previous experience working within a case management role and have worked or have knowledge of the insurance sector.

Our client requires case managers who have an understanding of the value of integrating medical services and return to work activities within a client’s rehabilitation program. The role will involve:

• Conducting Immediate Needs Assessments with clients face-to-face in their homes;
• Assessment of a client’s physical and psychological capacity; identification of barriers to recovery; and development of effective rehabilitation plans;
• Communication amongst clients, medical and health practitioners, and other stakeholders;
• Vocational rehabilitation – returning clients to meaningful work with their same or new employer;
• The coordination of treatment programmes;
• Rehabilitation reporting.
